First, we’ll ask you to share your concerns and insight for your family member. Then, our care manager will visit and conduct a thorough on-site assessment to determine the current and future needs necessary to enhance your love ones life. Consider this a “healthcare balance sheet”.
Our care managers know how to get to the relevant issues as sensitively and unobtrusively as possible. The assessment will provide an in-depth analysis of your family members current status. This is accompanied by a detailed care plan with recommendations for cost-effective services and resources. All information and recommendations are strictly confidential protecting you and your loved ones privacy.
Due to COVID-19 and the need to protect our clients and our team members. PPE will be utilized for all visits. If necessary we are able to complete an assessment virtually if the client has access to audio and video services.
