RN Life Care Advocates is a trusted resource for professionals who value a strategic approach to life and healthcare solutions. An Aging Life Care Professional®, Certified Care Manager (CMC) is the new team member alongside the medical team, attorney, and fiduciary advisor to ensure the individual’s desires and needs are met.

Our care managers are advocates for your clients who are unable to effectively advocate for themselves. We provide unbiased, independent assessments of quality-of-life needs and goals.

Retaining a care manager optimizes resources and improves the quality of life for the aging or disabled individual. We have extensive knowledge about the cost, quality and availability of resources in each community we serve. Our professional care managers have exceptional written and oral communication skills. They’re great listeners and have the skills to manage family emotions when there are care issues or a crisis that needs to be addressed.

We coordinate and assess the selection of health care providers, ensure quality and effectiveness of care, and provide collaborative planning and support. Our care managers attend medical appointments to ensure continuity of care, accuracy and completeness of information to the providers, virtually and in person.
