An Aging Life Care Professional® (ALCP), also known as a geriatric care manager, is a health and human service specialist who acts as a guide and advocate for families who are caring for an older relative, and/or disabled individual.

Aging Life Care Professionals® help manage and coordinate the challenges when an individual has experienced a decline in health or cognitive changes that are impacting their daily life. We are the eyes and ears to the families of our clients, assisting them in navigating the continuum of care and the nuances within each with the goal of preventing a crisis. Aging Life Care Professionals® become the “coach” and the families and clients the “team captain.”

Our Aging Life Care Professionals®  are Registered Nurses and Care Manager Certified (CMC) through the National Academy of Certified Care Managers. They have extensive knowledge of the medical, psychosocial, cognitive, and physical challenges of aging and disability. We are trained to know how to navigate complex systems and explore cost effective resources while providing support and advocacy.

RN Life Care Advocates does not maintain a vested interest in any health care provider organization. As an objective third-party, we assess and identify what services your family member needs to remain independent and safe as possible.

As Aging Life Care Professionals® we make sure that when you can’t be with your loved one, someone will be, providing the same level of care and concern that you would if you could be there.
